Carepath’s Chronic Disease Management Program

Carepath’s Health Care Navigation program provides individualized case management for all medical conditions. A single point of contact with one Nurse Case Manager maintains continuity in care.

All services are delivered virtually via telephone or through secure video conferencing facilitating timely and efficient support. Your Nurse Case Manager is here to assist you through diagnosis, treatment and plan of care. The program works in parallel to the public health care system and does not replace your current health care team.

How the program works:

When you call the toll-free number you will be connected with our intake team who will ask you some general questions. An email is then sent with a link to the program consent forms. Upon completion of the documents, a Nurse Case Manager will contact you within 1-2 business days. This is an opportunity for the nursing team to answer any questions regarding the navigation program, conduct a thorough review of your medical history, current diagnosis, associated symptoms, and to discuss any other questions you may have about your condition.

Upon completion of the navigation your Nurse Case Manager will provide a comprehensive summary of the resources provided during your journey. An optional Carepath Satisfaction Survey used to improve the delivery and experience of our services will also be sent out.
